Advies behandeling indien geen deelname aan studie

3 x R-CHOP21 + IF-RT of 6 x R-CHOP
Stadium II-IV
< 65 jaar
3 x R-CHOP21 gevolgd door 2 kuren hoge dosis cytarabine plus rituximab, gevolgd door stamcelmobilisatie, consolidatie (BEAM) met autologe stamceltransplantatie
 
> 65 jaar
6-8 x R-CHOP21

 

Primair refractaire ziekte en recidief
Vrijwel alle patiënten zullen vroeg of laat progressie dan wel recidief van de ziekte ontwikkelen.
Voor palliatie zijn meerdere schema’s in gebruik, de keuze hangt onder andere af van de eerstelijns behandeling. Indien nog geen R-CHOP is gegeven is dit een goede optie. De combinatie Rituximab, fludarabine en cyclofosfamide (R-FC) is een andere mogelijkheid. Ook de combinatie dexamethason- rituximab- cytarabine, rituximab- bortezomib of temsirolimus kan worden overwogen. Ook R-PECC (rituximab, prednison, etoposide, lomustine, chloorambucil) of chloorambucil lage dosis continu zijn schema’s die palliatief gebruikt kunnen worden.
Voor een selecte groep patiënten kan in de 2e lijn na reinductie behandeling een allogene stamceltransplantatie overwogen worden. Overleg hiervoor met de consulent van het centrum
Ook voor inclusie in fase I-II trials is overleg met het centrum zinvol.
 
Een enkele patiënt heeft een indolent verlopend mantelcellymfoom. Een expectatief beleid is dan verantwoord.
